Program of All-inclusive Care for the Elderly (PACE)

It’s hard to know how to best help a person who is aging. We want the best for them, but finding or giving the right care, and getting other resources to help can be difficult, overwhelming and tiring.

  • How do I make sure their medications are well managed?
  • How do I get them to all their doctor appointments?
  • How do I make sure all the doctors are coordinating my loved one’s care?
  • What do I do if I have to leave my loved one at home alone? Who will look after them and keep them safe? Or keep them from growing lonely or bored?

Your loved one may be able to get the care they need AND remain in the home they love. PACE (Program of All-inclusive Care for the Elderly) is a unique kind of care that has helped thousands of seniors continue living in the community.

CarePartners PACE (Program of All-inclusive Care for the Elderly) is a program for people age 55 or older that need support to remain safely at home. It provides community-based care and services for people who might otherwise need a nursing home level of care. A PACE program works to promote and sustain the independence of seniors, and help those who want to live at home, stay at home.

CarePartners PACE services include, but are not limited to:

  • Primary care (including doctor and nursing services)
  • Hospital care
  • Medical specialty services
  • Prescription drugs
  • Nursing home services
  • Emergency services
  • Home care
  • Physical therapy
  • Occupational therapy
  • Adult Day Care
  • Recreational therapy
  • Meals
  • Dentistry
  • Nutritional counseling
  • Social services
  • Laboratory/X-ray services
  • Social work counseling
  • Medical transportation

All services, except emergency services, must be authorized by CarePartners PACE or the participant may be fully liable for the cost of unauthorized services.

Our Interdisciplinary Team of professionals develops and coordinates the care of each participant enrolled, based on his or her individual needs by developing a Plan of Care for the senior…the goal is always to help the person remain living in their community, by keeping the individual as healthy as possible and giving them the best quality of life they can have!

Who is eligible? A person is eligible for CarePartners PACE if they:

  • Are 55 years or older
  • Live within the service area (Buncombe or Henderson County)
  • Meet the North Carolina Nursing Home Level of Care Criteria
  • Are able to safely live in the community at the time of enrollment with PACE services

All CarePartners PACE services are paid for by Medicare, Medicaid and/or private pay. If you are fully eligible for Medicaid, there is no cost.
