This is the experience everyone should have at the end of life. It is what hospice care is all about. When you or someone you love is facing a terminal illness, CarePartners Hospice Services is here to provide physical, emotional and spiritual support.

Hospice care providers work to improve the quality of our patients’ lives, helping them to live their last days, weeks or months as free of pain and other distressing symptoms as possible

  • Medical care for people with an anticipated life expectancy of 6 months or less, when cure isn’t an option, and the focus shifts to symptom management and quality of life.
  • An interdisciplinary team of professionals trained to address physical, psychosocial, and spiritual needs of the person; the team also supports family members and other intimate unpaid caregivers.

Continued Support for Loved Ones

Support for grieving family and friends is provided as part of the CarePartners Hospice program. Services include professional grief counseling, grief education classes, support groups for adults and children, and memorial services.

Making the Decision to Enter Hospice Care

At CarePartners Hospice we offer care to all ages, from the very young to the oldest members of our community. Palliative care is also available for patients who are not yet ready for hospice. You can read more about eligibility for hospice care here.

Did You Know...

Hospice care helps patients and families manage care of someone with a terminal illness. It’s neither giving up nor hastening death. The goal of hospice support is to increase patient comfort as much as possible. Palliative care, different from hospice, is available to help manage chronic, often declining, health problems for patients who are perhaps not ready for hospice.
